Pontocerebellar-hypoplasia
Autosomaalinen resessiivinen
Aiheutuu muutoksista molemmissa TSEN54- tai MINPP1-geenin kopioissa. TSEN54 on osa entsyymiä, joka käsittelee siirto-RNA:ta, ja yleinen muutos p.Ala307Ser löytyy pontocerebellar-hypoplasian tyypistä 2. MINPP1-muutokset aiheuttavat erillisen muodon, tyypin 16.
Oireet ja merkit
Vaikea kehitysviive, jolla motorinen kehitys on vähäistä tai puuttuu kokonaan, etenevä mikrokefalia sekä syöttämis- ja nielemisvaikeuksia jo vauvaiästä alkaen. Tahattomat liikkeet (dystonia tai korea) ja kohtauksia ovat yleisiä. Monilla lapsilla elinajanodote on lyhentynyt.
Mitä diagnoosi muuttaa
Hoito on oireenmukaista: ruokinnan tukeminen, kohtausten ja liikehäiriöiden hoito, fysioterapia ja palliatiivinen hoito. Geneettinen neuvonta selittää resessiivisen periytymisen, ja perheille on saatavilla kantajuus- ja synnytystä edeltävä testaus.
