Congenital Adrenal Hyperplasia
- Autosomal recessive
Congenital adrenal hyperplasia (CAH) is an inherited endocrine disorder caused by a steroidogenic enzyme deficiency that is characterized by adrenal insufficiency and variable degrees of hyper or hypo androgyny manifestations, depending of the type and the severity of the disease.
- 4 GenCC submitters rate it definitive or strong
What knowing changes
Glucocorticoid replacement (hydrocortisone in children, prednisone/dexamethasone in adults); mineralocorticoid replacement for salt-wasting (fludrocortisone); adrenalectomy for severe medically refractory hyperandrogenism; stress dosing protocols
Monogenic Hypertension & Mineralocorticoid Disorders
- Familial HyperaldosteronismAutosomal dominant
A group of conditions, including glucocorticoid-remediable aldosteronism (CYP11B1/CYP11B2) and forms caused by CLCN2, KCNJ5 or CACNA1H, in which the adrenal glands make too much aldosterone, causing high blood pressure.
- 1 GenCC submitter rates it definitive or strong
What knowing changes
Knowing about it guides treatment (dexamethasone for glucocorticoid-remediable aldosteronism, aldosterone-blocking drugs more generally) and supports screening of relatives.
Inheritance
- Autosomal dominant
- One changed copy, from either parent, is enough. A parent who has one passes it on to each child with a one-in-two chance.
- Autosomal recessive
- It takes a changed copy from each parent. With one, you are a carrier and usually unaffected; when both parents are carriers, each child has a one-in-four chance of inheriting two.

Exons of the gene's MANE Select transcript (release v1.5), from NCBI and EMBL-EBI, on the GRCh38 reference genome.
Chromosome bands from UCSC's GRCh38 ideogram (2022-10-29).
Nearby genes from Ensembl release 116, on GRCh38.
Exons make up 3.5 kb of the gene's 7.5 kb; 1,512 of those bases code for the protein.
How a genome test reads CYP11B1
148
positions in CYP11B1 where ClinVar classes a change as pathogenic or likely pathogenic
148 of them (100%) fall in regions short reads find hard.
- Low mappability (67 sites)Regions where 100 bp or 250 bp reads cannot be placed uniquely.
- Segmental duplications (148 sites)Copies of at least 1 kb sharing at least 90% identity with another part of the genome (paralogues, pseudogenes); reads may align to the wrong copy.
A whole genome covers all 7.5 kb of CYP11B1, introns included, not only the 3.5 kb of exons.
