Alkaline phosphatase is a ubiquitous enzyme that catalyzes dephosphorylation reactions, ie the "removal" of phosphate groups from certain substrates. It is particularly present in the liver, bones, intestines and placenta (pregnant women), in the form of four tissue and non-tissue-specific isoenzymes encoded by as many gene loci. It represents an important clinical marker of hepatic and bone pathologies, but the increase in its serum concentration can also be found in a benign and transient form during growth or following infectious processes of a viral nature. There is also a non-pathological form of hyperphosphatasemia, which manifests itself with a constant increase in alkaline phosphatase in members of the same family, in the absence of clinical conditions justifying the increase. This is a rare and inherited metabolic disorder, the exact origin of which is unknown.
As anticipated, this enzyme is particularly abundant in certain areas of the body, with the following isoforms: ALPI (intestinal form), ALPL (non-tissue-specific form present in liver, bones and kidneys), ALPP (placental form) and ALPG (germinal form).
The intestinal form is expressed in the cells of the brush border and participates in the antimicrobial defenses.
Its contribution to the total serum alkaline phosphatase concentration is lower than the bone and hepatic isoform, which represent the majority shares (about 80%) and are those of greatest medical interest.
Bone alkaline phosphatase reflects osteoblastic activity and intervenes in bone mineralization, while in the liver it is expressed in the cells lining the bile ducts.
Placental isoform is found in pregnant women, but its possible ectopic expression may be related to ovarian neoplasia.
Finally, germinal alkaline phosphatase, closely associated with placental and intestinal phosphatase, is found in the testes, thymus and some germline tumors.
